If you need access to a dependent or loved one’s ProMedica MyChart Account, you will need to complete the MyChart Proxy Access form. Completed forms can be sent via email, fax (419-479-6919), or dropped off at any one of our ProMedica facilities. Please be aware that information sent via email is not secure and could be misdirected or intercepted in transmission.
